For the central patients, make the most involved side the ipsilateral one

There's an issue with that: In my models, it is possible to add only a one-sided diagnosis to a patient. This means, a patient's list of diagnoses is not necessarily symmetric and I cannot always make this comparison of which side is more involved. So I'm not sure if this makes a lot of sense...

Since I have migrated from right & left reporting to ipsi & contra, this is no longer relevant
