Feature Description

The current Aldine contact form has four required fields: name, email, institution, and message. Some network manager would like to customize the labels for these fields and make some of them optional. For example, they may wish to change the 'institution' label to 'division' or 'department' or 'role' and make it optional. Giving network managers some tools to customize the contact form in Aldine's customizer would satisfy this need.

An enterprise network manager has requested this feature as well in a recent ticket. They would like to be able to add questions to the form.

As network manager, I keep getting messages with no context. They think I know what book they were looking at and what page they were on when they typed their message. I can't help them if I don't have this information.
