D1 is a zener protection diode on input voltage, but the power rating is unsuitable for this application, where voltage spikes easily exceed the minimum threshold of 5.2V.

The diode needs to be changed to a more robust type. According to the datasheet, there is probably no requirement to be the zener type, because the SW pin windstands up to 23V and is also the output. Therefore the diode protects only a polarity in the situation where someone connects the voltage to the regulator output.
