OPC UA rust is going to move to a single crate model

I have a single-crate branch for 0.10 which will move from a multi crate model (opcua_core, opcua_crypto, opcua_types, opcua_server, opcua_client, opcua_console_logging) to a single opcua crate. This hopefully makes it easier to build the crate, reduces the potential for duplicate dependencies, speeds up the build and easier to publish.

I intend to land in the next while (previously few days) but I'll see if I can resolve some of the longstanding pull requests because files will move around a bit.

Instead of crates you can use features to turn on or off things, e.g.

version = "0.10.0"
features = ["server", "console-logging"]

Okay it's done. I took in a few pulls prior to this in master and I intend to bump from 0.10 to 0.11. So 0.10 is literally just rearranged sources
