Update Galveston testbed notebook with combined damage and epf damage

There are new and updated analyses that can be added to the Galveston testbed notebook. These include:

1. Update the testbed notebook to include building damage for wind, surge-wave and flood
2. Adding the combined building damage analysis and chaining the building damage to it
3. Adding EPF damage
4. Updating population dislocation to use the combined damage analysis output

Moving this to 4.2.0. It's not clear if we need to continue to maintain this testbed notebook or wait for the researchers updated notebook.
